  • Description
  • Provide coordination and supervision for the health care needs of individuals receiving services. To be an advocate for the individual and ensure all health and safety needs are met with dignity. Provide mentorship, guidance, and training to employees/contractors who provide direct support.
  • Contribute to the evaluation of the individual’s progress and provide input to the team regarding the health care of the individual.
  • Create health care plans to meet the individual’s needs. Provide training on all plans to employees/contactors providing support.
  • Compete annual (or as indicated) nursing assessments on individuals supported.
  • Utilize Therap as directed by state and agency standards.
  • Review written and verbal reports/documentation related to all medical and health related needs/findings and provide intervention or training when needed.
  • Provide agency oversight of all medication management. Including pharmacy licensure and quarterly pharmacy reviews.
  • Provide after hours and weekend on call support.
  • Work within the requirements and guidelines of the NM nurse practice act.
  • Conduct regular audits of medication storage and management in each location holding a pharmacy license.
  • Track all medical appointments and review physician consultation notes and provide follow-up on all recommendations.
  • Provide monthly review and updates for each individual’s MAR.
  • Provide billing updates, documentation for services requiring nursing on a quarterly basis or as agreed upon with the agency.
  • Respond to employees and contractors regarding any illness or injury of an individual. Provide direction and guidance through prudent nursing judgment. This may include a face-to-face nursing assessment at the time of the report.
  • Review all GER events in Therap within 24 hours of notice
  • Provide emotional support and feedback to the individual and their family if applicable.
  • Evaluate employee/contractor competency of individual diagnosis, health plans, and all recent changes in conditions. Provide formal and informal feedback and training to the employee/contractor.
  • Complete T-logs corresponding with billable services and all assessments (annual and as needed)
  • Complete all required training and work within all HeartWell Policies & Procedures.
  • Maintain regular communication with the Service Coordinator, Assistant Program Director, Program Director and HR on all appropriate related matters.

  • On-call nursing
  • Track available nursing units
  • Participation in the HRC Committee
  • Support and assist in the use of Station MD
  • Fully support, train and uphold all AWMD policies
  • Requirements
  • Current Nurse licensure
  • Current and Valid Driver’s License
  • Safe insured/ registered vehicle
