• Key Responsibilities

• * Provide skilled, in-home nursing care to assigned patients
• * Develop and follow individualized care plans
• * Accurately document patient care, observations, and updates
• * Collaborate with interdisciplinary care teams
• * Maintain compliance with DOL, CMS, and state regulatory requirements

• Qualifications

• * Current RN license in the state of [NM]
• * Current CPR certification
• * Valid driver's license and reliable, insured vehicle
• * Home health experience preferred but not required
• * Must pass pre-employment screenings
