  • As a Hospice Nurse Practitioner with Corus Health, you step into moments that require presence, steadiness, and compassion.   • This role is ideal for an experienced hospice NP who thrives in dynamic clinical environments, values meaningful patient relationships, and brings both empathy and clinical expertise to every visit.
  • Key Responsibilities
  • Perform comprehensive assessments, manage symptoms, and respond to rapid changes in condition using hospice and palliative best practices.
  • Complete required face-to-face hospice recertification visits and contribute to terminal prognosis documentation.
  • Partner closely with physicians, nurses, social workers, and chaplains during IDG meetings to support coordinated care plans.
  • Provide education on prognosis, medications, advance directives, and goals-of-care conversations with clarity and compassion.
  • Document all clinical encounters accurately, timely, and in full compliance with CMS and hospice regulatory standards.
  • Guide patients and families through end-of-life decision-making, offering emotional steadiness and support.
  • Work autonomously in the field while maintaining open, timely communication with the clinical team.

  • What We’re Looking For
  • Current Nurse Practitioner licensure in the state of practice and certification as FNP, AGPCNP, or equivalent.
  • Prior hospice NP experience is required (minimum level necessary for independent field work).
  • Strong clinical judgment with the ability to manage complex symptoms and evolving patient needs.
  • Demonstrated ability to navigate difficult conversations with warmth, composure, and honesty.
  • High-quality documentation skills and comfort working within regulatory requirements.
  • Steady, empathetic presence — especially in emotionally intense situations.
  • A team-oriented professional who values collaboration, transparency, and disciplined follow-through.
