Southwest Cancer Care, a leading Cancer Center based in Arizona, is expanding its team and seeking a Clinical Navigator to join us remotely. As our patient volume continues to grow, we are looking for a dedicated and skilled individual to support our clinical team and ensure our patients receive the highest quality care. This is an exciting opportunity for healthcare professionals who are passionate about making a difference in the lives of cancer patients. You will work remotely, collaborating with our physicians, nurse practitioners, and administrative staff to provide comprehensive care and seamless communication for our patients. Key Responsibilities: - Patient Follow-up: Regularly reach out to patients to monitor their symptoms, address concerns, and provide support under the guidance of physicians and Nurse Practitioners. - Clinical Triage: Identify and triage clinical issues, escalating them to the appropriate healthcare provider as necessary. - Patient Scheduling: Schedule treatments, including injections, infusions, and lab tests, ensuring smooth coordination between care teams and patients. - Team Coordination: Work closely with both the Front Office and Back Office teams to streamline patient care and operations. Required Qualifications: - A healthcare-related degree (required) - RN (Registered Nurse) preferred and ideal - MD (Medical Doctor) also considered - Strong communication skills and the ability to work independently in a remote environment Additional Information: - Comprehensive training and shadowing will be provided to ensure success during the onboarding process and beyond. - This is a Full-Time position and we value long-term commitment, with many of our team members staying with us for over 4 years (we opened about 5 years ago).
